While participating in this project, I consent to the following:

I declare that I am 18 years or older, OR

I verify that my child is at least 15 years of age.

I/my child consents to being contacted by RAC for the purpose of this project.

I consent for me/my child to be included in photographs and videos that are taken by RAC on the day, and I consent for RAC to use the photographs and video content of them in RAC advertising or promotional materials

I acknowledge that I/my child is participating on a volunteer basis without any pay or compensation of any kind on behalf of RAC or RAC’s project partners.

I/my child agrees to participate in all activities in a safe and reasonable manner and will follow the instructions of workshop facilitators.

I/my child understand/s that all activities are undertaken at my/their own risk.
